Whether wine, beer, or cocktails, alcohol is a given for many people. However, even relatively small amounts could play a larger role for women than previously thought. This is highlighted by a recent analysis of scientific studies.

Meta-analysis evaluates data from more than 2.5 million women

For the analysis published in the journal “The Breast,” researchers compiled results from 37 studies involving a total of 2,565,920 women. The result: Women who consumed alcohol had a 17 percent higher risk of developing breast cancer compared to those who did not drink alcohol.1

Particularly notable: The risk increased with the amount of alcohol consumed. Even up to about ten grams of alcohol per day–roughly equivalent to a small glass of wine–was associated with a 13 percent higher risk of breast cancer. With a daily intake of about ten to 20 grams of alcohol, the risk rose to 28 percent. Women who consumed more than 20 grams of alcohol per day had a 52 percent higher risk.

Risk increases with alcohol consumption

The authors, including Brussels cancer specialist Dr. Luca Arecco, speak of a clear link between alcohol consumption and breast cancer: The more alcohol consumed, the higher the risk of developing the disease. The analysis did not indicate any amount of alcohol that could be considered safe.

The connection was particularly evident in breast cancer types whose growth is promoted by female sex hormones. For other types of breast cancer, the researchers found no statistically significant link.

Why might alcohol increase breast cancer risk?

The meta-analysis itself does not investigate the biological causes. However, the authors mention several possible explanations already known from previous studies.

When alcohol is broken down, acetaldehyde is produced–a toxic metabolic product that can damage the genetic material of cells. Additionally, alcohol can promote so-called free radicals, which also cause cell damage. Furthermore, alcohol consumption can increase the levels of female sex hormones like estrogen. Since many types of breast cancer are influenced by these hormones, this could further increase the risk of the disease.

Breast cancer is the most common cancer among women. According to the study authors, about one in eight women will develop it during their lifetime. In addition to factors such as age or genetic predisposition, lifestyle factors are also considered modifiable risk factors. These include, among others, being overweight, smoking, and alcohol consumption.

No evidence of worse disease progression

The researchers also examined whether alcohol affects the further course of the disease if breast cancer has already been diagnosed. Here, a different picture emerged. The analysis found no evidence that alcohol consumption increases the risk of breast cancer recurrence or death from breast cancer.

Although the scientists found a slightly higher overall life expectancy among women with light to moderate alcohol consumption, the authors explicitly emphasize that no health benefit of alcohol should be inferred from this. The observed differences could rather be due to other factors, such as differences in diet, exercise, income, or medical care among the women studied.

What the study reveals–and what it doesn’t

The new meta-analysis is among the largest evaluations to date on the link between alcohol and breast cancer. Meta-analyses are considered particularly meaningful because they summarize and jointly evaluate the results of many studies.

Nevertheless, this study cannot prove causation. Most of the included studies were observational, where participants self-reported their alcohol consumption. Such reports can be inaccurate. Additionally, the studies differed in how they defined light, moderate, or heavy alcohol consumption.

Despite these limitations, the authors see their results as further evidence for limiting alcohol consumption as much as possible. Across all the studies evaluated, a consistent pattern emerged: As alcohol consumption increased, so did the risk of breast cancer.
