Actor Nicholas Brendon, best known as Xander from the cult series “Buffy the Vampire Slayer,” was only 54 years old. He “passed away peacefully in his sleep of natural causes,” his family announced on March 20. It is known that Brendon had faced several serious health issues simultaneously in the past.
“With deep sorrow, we announce the death of our brother and son, Nicholas Brendon. He died peacefully in his sleep of natural causes,” his family wrote on Saturday on Instagram. “It is no secret that Nicholas had struggled with difficulties in the past. He was on medication and undergoing treatment to manage his diagnosis and remained optimistic about the future until his death,” the statement published in the “Hollywood Reporter” continued.
Multiple Serious Illnesses Simultaneously
In 2023, Brendon detailed in an Instagram post the health issues he was dealing with simultaneously: In 2022, he had a heart attack, and a congenital heart defect was diagnosed. “After two spinal surgeries and a heart attack, I had numerous doctor appointments to coordinate, and dealing with health insurance and pre-approvals is almost as emotionally exhausting and painful as treating my actual injuries,” he wrote at the time on Instagram.
Cauda Equina Syndrome
The two spinal surgeries were reportedly related to his Cauda Equina Syndrome: This is a severe nerve compression in the lower back, usually due to a herniated disc in the lumbar spine. Cauda Equina Syndrome occurs when two or more of the 18 nerve roots below the conus medullaris are compressed or damaged, disrupting the motor and sensory pathways to the lower extremities and bladder. Emergency surgery is required if the syndrome causes, for example, urinary retention or incontinence and/or weakness of the lower extremities.1
It became known that Nicholas Brendon suffered from Cauda Equina Syndrome when he had to cancel appearances in 2022 due to health issues. According to his manager, Theresa Fortier, at the time, Brendon suffered from paralysis of the genitals and legs. An emergency surgery reportedly took place. She also claimed he had difficulty walking due to profound health issues and immense pain, which later necessitated a major back surgery.
Emergency Room After Heart Racing
After the second back surgery, the former “Buffy” star reportedly suffered cardiac arrest due to tachycardia (heart racing). His manager said this to the “Los Angeles Times.” In tachycardia, the heart beats too fast. The rate is then more than 100 beats per minute, in extreme cases up to 400 beats per minute. The racing heart is no longer able to efficiently pump oxygen-rich blood through the body.

Drug Problems and Mental Illness
Brendon also spoke openly about his drug problems and mental illness, which sometimes led to legal troubles.2 From his thirties, the “Buffy” star publicly struggled with substance abuse and mental health issues.
His life was overshadowed by health and personal crises—yet Nicholas Brendon remains remembered by many as part of one of the most influential series of the ’90s.
