For women, aging takes a noticeable leap with the onset of menopause. In comparison, the aging processes in men are slower and more continuous. However, changes also occur in men over their lifetime, including in the intimate area. FITBOOK consulted an expert to find out how aging affects male genitalia.
What Aging Phases Do Men Go Through?
Unlike female menopause, hormonal aging in men occurs gradually. Nevertheless, several broad phases can be distinguished.
Early 30s
In their 20s and early 30s, the body is usually at its hormonal peak. Testosterone levels are high, energy levels, muscle growth, and libido are generally pronounced, and first changes in the intimate area are extremely rare during this phase.
Mid-30s Onward
From around the mid-30s, testosterone levels begin to slowly decline. This process is often medically referred to as “andropause” and compared to menopause, although it is much less abrupt than women’s menopause. Now, initial changes may appear in the form of decreased regenerative ability, less stress resistance, and fluctuating libido.
From Age 40
Between 40 and 60 years, these effects often become more noticeable. Some men report decreased sexual desire, reduced erection hardness, or longer recovery periods after sex. At the same time, weight gain, muscle loss, and a slower metabolism may occur.
From Age 60
From around 60 years, hormonal changes intensify further, and blood circulation and nerve function may also decline. Nevertheless, men can remain sexually active into old age.

How Do Male Genitalia Change?
Skin and Elasticity
“As you age, the skin changes all over the body, including the intimate area,” explains Dr. Christoph Pies, a urology specialist. “The skin on the penis and scrotum becomes thinner and loses elasticity, which can make it appear somewhat looser overall. One reason for this is the declining collagen production, which is responsible for firmness and strength.”
Circulation and Appearance
Over the years, circulation can also decrease, causing the penis to appear smaller or less full in a flaccid state. A slight lightening of the skin is also possible, as it is less well-supplied with blood. These changes are generally harmless but can feel unfamiliar.
Intimate Hair
Just like on the head, body hair density can decrease with age, hair can become finer, or its structure can change. In some cases, it grows more slowly or even falls out in places. This development varies individually and is strongly influenced by genetic factors.
Skin Condition
Sebum production often decreases with age, which can make the skin drier and more sensitive to friction or external influences.
Odor
The natural odor can also change slightly, which is partly due to altered skin conditions and the natural bacterial flora. Adjusted intimate care can help maintain a pleasant skin feel and prevent irritation.
Can the Penis Get Smaller with Age?
“This question concerns many men, and there is indeed some truth to it,” says Dr. Pies. “With age, the penis can indeed appear somewhat smaller. There are several reasons for this. One of the most important is reduced blood flow, as less blood reaches the erectile tissue, making the penis appear less voluminous. This applies to both the flaccid and erect states. Another factor is the change in fat tissue. Especially in the lower abdomen, fat can accumulate, covering part of the penile shaft. This makes the penis appear shorter, even though its actual length has not significantly changed. In rare cases, conditions such as penile curvature (Peyronie’s disease) can also lead to shortening. Consider slight changes as normal, but larger changes should always be medically evaluated.”
Is Ejaculation Possible at Any Age?
In principle, the ability to ejaculate remains intact even in old age, although the process and intensity may change. Many men notice that the amount of ejaculate decreases over the years. This is partly because the production of seminal fluid decreases and the contraction strength of the muscles responsible for ejaculation diminishes. The so-called refractory period, the time the body needs to become aroused again after ejaculation, can also lengthen. While this period often lasts only minutes or hours in younger years, it can be days in older age. In some cases, changes such as delayed or absent ejaculation can occur, with both physical and psychological factors playing a role.
Can Men Get an Erection at Any Age?
“Erections depend heavily on good circulation and functioning nerves. Both can be impaired with age. As a result, it may take longer for an erection to occur, and it may be less firm or stable than before,” explains Dr. Pies. “External factors such as stress, medications, cardiovascular diseases, or diabetes can also affect erectile function. However, this does not mean that fulfilling sexuality is no longer possible in old age.”
Change in Sexual Drive
Sexual drive, or libido, is closely linked to the hormone testosterone. As its levels decrease with age, sexual desire may also decline. However, libido is not purely a biological phenomenon, as psychological factors, relationship quality, stress levels, and overall well-being also play a significant role. Some men report a significant decrease in desire, others experience little change. Still, others find their sexuality in old age to be more relaxed and conscious, as performance pressure and insecurities decrease.

Exercise
While many changes are part of the natural aging process, there are several things you can actively do for the health of your genitalia. A key factor is overall physical health. Regular exercise promotes circulation and supports vascular health, both crucial for erectile function. Endurance sports, strength training, and an active lifestyle have positive effects here.
Nutrition
A balanced, nutrient-rich diet can help reduce inflammation and protect blood vessels. Foods with antioxidant properties and healthy fats play a significant role.
Weight Control
Overweight, especially abdominal fat, can negatively affect hormonal processes and endanger health.
Mental Balance
Stress, performance pressure, or anxiety can directly impact libido and erectile function. Relaxation techniques, adequate sleep, and a conscious approach to stress are therefore advisable.
Preventive Checkups
“The be-all and end-all for your health are regular preventive checkups. Only in this way can conditions such as high blood pressure, diabetes, or prostate enlargement be detected and treated early,” advises Dr. Pies.
Can Hormone Preparations Help Halt the Changes?
“Hormone preparations, especially testosterone therapies, are often discussed in connection with age-related changes in men. Indeed, a medically indicated testosterone replacement therapy can help improve symptoms such as reduced libido, lack of drive, or muscle loss in the case of a proven deficiency. Indirectly, this can also positively affect sexual function and body image,” says Dr. Pies. “However, hormone preparations are not a general ‘anti-aging remedy’ for the genitalia. They cannot completely halt natural aging processes such as skin changes, reduced elasticity, or decreased circulation. Also, potential side effects, such as on the cardiovascular system or the prostate, should not be underestimated. Their use should always be individually considered and only after medical diagnosis and under regular monitoring.”
