Actress and comedian Rosie O’Donnell has undergone a significant transformation. Initially, the American presented herself a year ago as slimmer than ever. Now, her face has also changed dramatically. FITBOOK knows what’s behind the 64-year-old’s transformation.
For years, Rosie O’Donnell struggled with her weight and health issues. But now, the pounds have melted away for the U.S. TV icon. The new slim figure is the result of lifestyle changes, surgery, and weight-loss medications, including a weight-loss injection. But that’s not all. O’Donnell also underwent facial surgery–which visually rejuvenated her by about 15 years.
Her Face–Before and After
Quietly and discreetly, the 64-year-old went under the knife for a facelift. A step she explained on the platform “Substack.” Because: She used to be against such procedures and didn’t hold back her opinion. “I thought it was a betrayal. Of feminism. Of the global team of us women,” but then something changed: “And then I lost 50 pounds…”
Finally, she was slimmer and living healthier, but she no longer felt comfortable in her skin. “Those weren’t wrinkles–that was gravity. I looked in the mirror and thought: This isn’t aging, this is a conscious melting away,” O’Donnell wrote.
On the show “SiriusXM’s Radio Andy,” she revealed that after the weight loss, she had a lot of excess skin on her neck. People constantly asked if she was sad. Eventually, she warmed up to the idea of getting a facelift and looking 15 years younger. O’Donnell showcased the results on Instagram and has proudly displayed them at public appearances since.
A Year Ago, Rosie O’Donnell Surprised with a Slim Figure
On her social media account, Rosie O’Donnell posted a photo in May 2025 that caused a stir. In the screenshot, where she showed herself on stage, she appeared trim and slim. “I can’t believe this is me now,” she captioned the photo in amazement. She also added the hashtags “#mounjaro,” “#weightloss,” and “#bodydismorphia,” directly revealing what had significantly helped with her weight loss.
How Mounjaro Works
As early as January 2023, Rosie O’Donnell revealed that she had been using the now-popular weight-loss injection Mounjaro among U.S. celebrities since late 2022. The medication, prescribed by her doctor for Type 2 diabetes, reduces appetite and improves or accelerates the breakdown of fat and sugar in the body. The reason is the active ingredient Tirzepatide. Similar to Ozempic, Mounjaro also prolongs the feeling of fullness.

Size L Instead of XXL–Consequences of a New Lifestyle
In March of the past year, Rosie O’Donnell gave an update on her life with Mounjaro via her TikTok account. “I’ve lost more weight. I take Mounjaro for my diabetes, and one of the side effects is weight loss,” she said there. Additionally, after moving with her autistic son Clay from Los Angeles to Dublin in January, she no longer has a cook and now cooks for herself. This has also positively impacted her weight.
The effects in everyday life were quickly noticeable. The actress lost so much weight that her old clothes no longer fit. “I’ve always been one of those people who had a weight problem. And now that I’m a size L–instead of XL or XXL–I find it shocking,” she shared in amazement.
Rosie O’Donnell Has Been Using the “Weight-Loss Injection” for Three Years
Her weight-loss journey with Mounjaro began about four years ago. Since then, she has also adjusted her diet. She mentioned that she consumes as little sugar and diet colas as possible, but drinks more water. “And I try to move more. All these things together, that’s it,” she said on TikTok.
Slimmer “Thanks” to Heart Attack
However, Rosie O’Donnell is by no means one of the celebrities who just want to lose weight due to Hollywood’s obsession with thinness. No, the truth is rather a serious and long medical history. In 2012, at the age of 50, she suffered a severe heart attack due to her significant overweight, which nearly cost her life. “I should have died,” she revealed in an episode of “The Best Podcast Ever” in September 2023.
Doctors then urged her to lose about 44 pounds. She tried for a year, and success came in small steps. To speed things up, she underwent a necessary gastric bypass in 2013. As a result, she lost an impressive 64 pounds within a year. “That surgery changed my life,” she revealed to the U.S. magazine “People” nearly ten years ago.
